Patient Fact Sheets

Patient Fact Sheets are broken up into steps. Also depends on if you have access to multiple fact sheets (English and Spanish).
If you have access to multiple then the first step would be to choose to Set Up either the English or Spanish Fact Sheet. If you only have access to one Fact Sheet then you are taken to the Select Account step.
Then you can choose an account:
After choosing the account then the user will need to go through a 3-step process. At this point, the 'Logo' is generated by using the full name and the organization name to fill out the logo. Users will need to upload the picture 'logo' that they want to be shown in the pdf. Double-check their address and finally review and accept the terms and conditions.
When choosing an Account the user can either choose themselves or an Organization that they are part of. The data is pulled from IMIS with an IQA calling Organization administrators. Once chosen the professional address is grabbed. If the user is chosen the Professional Address is the address on the user's account if the other Organization administrators are chosen then the Professional Address on that object is grabbed.
During the 3-step process, the user is not allowed to change their name or the organization name that shows up in generated logo; however, the address can be adjusted during step 2 if the user chose themselves.
Once submitted the user can view the Patient Fact Sheets and choose to Download each individually. There is also checkboxes that can be checked to download multiple by clicking 'Download Selected Files'
Once set up is complete if the user navigates back to the first screen then the fact sheet will have a view option instead of setup.
